INSURANCE AND FINANCING

Most insurance carriers do pay benefits for artificial eyes and scleral shells.  Check with your insurance representative about benefits for durable medical equipment.

L&B Laboratories, Inc. is a supplier of durable medical equipment limited to the supply of custom-made prosthetic (artificial) eyes.  We are NOT physicians.

We accept:

• VA Benefits
• Tri-Care
• Workers’ compensation

We are in network with/accept assignment with:

• Humana
• United Healthcare
• People’s Health
• Medicare (including most Medicare Advantage plans)
• Louisiana Medicaid

We are happy to complete pre-authorizations and file claims for any health insurance carrier, regardless of our network status, however the patient or guarantor is responsible for any unpaid balance at the time the prosthesis is delivered to the patient.  Payment is due in full when the prosthesis is completed.
